University Police -- Report Request
Form Individuals who wish to obtain a
copy of a UT Police Incident or Accident report must
fill out a Report Request form. The forms are available
at the UT Police Communications Center, in the
Transportation Center, or on line (below).
NOTE: If you need to FILE a
report, please contact the Police Dispatcher at
530.2600 Some things to
keep in mind when requesting a copy of a report:

Copies of reports cost $.50 (fifty
cents), and can be picked up 5 (five) business
days after this request is filed.

Reports that have not been approved by
a supervisor will not be released.

Reports pertaining to active
investigations may not be released.

If you would like to have a report
faxed you when it is available, you must stop by the
UT Police Department and pre-pay for the report.

If you would like to have the report
mailed, you must stop by the UT Police Department,
pre-pay for the report, and drop off a self-addressed,
stamped envelope.
Media outlets that
require reports or other information
should contact the Criminal
Investigations Bureau at 419.530.4438
Insurance companies
should contact the Records Clerk at 419.530.2001
Please fill out this form as completely as possible. The
more information you provide, the faster we will be able
to locate the original report and process your request.
